What is being tested?

previously named Campylobacter pylori, is a Gram-negative, microaerophilic bacterium found in the stomach. It was identified in 1982 by Australian scientists Barry Marshall and Robin Warren with further research led by British scientist Stewart Goodwin, who found that it was present in patients with chronic gastritis and gastric ulcers, conditions not previously believed to have a microbial cause. It is also linked to the development of duodenal ulcers and stomach cancer. However, over 80% of individuals infected with the bacterium are asymptomatic and it may play an important role in the natural stomach ecology.

More than 50% of the world’s population harbor H. pylori in their upper gastrointestinal tract. Infection is more prevalent in developing countries, and incidence is decreasing in Western countries. H. pylori’s helical shape (from which the generic name is derived) is thought to have evolved to penetrate the mucoid lining of the stomach.

جرثومة المعدة نوع من البكتيريا منالعصيات سلبية الغرام أليفة الهواء القليل التي تستعمر مخاطياتالمعدة والإثنا عشري، مسببة التهاباً في المخاطية، وترتبط بتطورالقرحات الهضمية في المعدة والإثنا عشري وسرطان المعدة. يبقى أكثر من 80% من المصابين بهذا الجرثوم بدون إي أعراض أو مضاعفات .

يحمل نحو 50% من سكان المعمورة هذا الجرثوم، مما يجعله العدوى الأكثر انتشاراً في العالم، وانتشاره أكبر في البلدان النامية منه في المتطورة. طريق العدوى غير معروف تماماً.

هناك عدة طرق لفحص H pylori

وقد قسمت الى طريقتين ;

♦non-endoscopy

a.non-endoscopy

1. Fecal antigen test
2. blood antibody test
3.urea breath test

b. Endoscopy →tissue biopsy sample obtained

1.Histology :Tissue examined.
2.Rapid urease testing .
3.Culture.
4.PCR .

Rapid urease test, also known as the CLO test (Campylobacter-like organism test)

How is it used?

is a rapid diagnostic test for diagnosis of Helicobacter pylori. The basis of the test is the ability of H. pylori to secrete the urease enzyme, which catalyzes the conversion of urea to ammonia and carbon dioxide

كيف اعمل الفحص ؟

The test is performed at the time of gastroscopy.

A biopsy of mucosa is taken from the antrum of the stomach, and is placed into a medium containing urea and an indicator such as phenol red.

The urease produced by H. pylori hydrolyzes urea to ammonia, which raises the pH of the medium, and changes the color of the specimen from yellow to red .

Results

RED→positive (+)
yellow→Negative (-)

هذا الفحص له عيوب ممكن اذا كان فيه

active gastrointestinal bleeding ruduces accuracy of the test

فيه دراسات لكن الي الان ليس لها مصدر تقول ان هذا الفحض حقه
Specificity and sensitivity of this test is high compared with histopathological and UBT
